It Is Not An Infection
Lichen sclerosus is an inflammatory skin condition. It has a strong preference for the vulva, and it is most common in women around and after menopause.
It is not fungal, which is why antifungal treatment does nothing. Three failed courses is not bad luck, it is a diagnosis pointing at itself.
The skin becomes thin, pale and fragile. White areas appear, with a shiny or slightly crinkled surface. It splits easily, which is where the fissure that keeps reopening comes from.

The Part That Does Not Come Back
Over time, untreated, it changes the architecture.

Two And A Half Million Women
Veerabagu and colleagues published in JAMA Dermatology in 2025, analyzing 2,525,340 female Medicare beneficiaries with a mean age of 73.
At least 0.7 percent had vulvar lichen sclerosus, a figure the authors state is likely an underestimate, partly because women do not raise these symptoms and are not asked about them.
Among the women who had it, 1.2 percent went on to develop vulvar squamous cell carcinoma or carcinoma in situ. Compared with women without the condition, the hazard ratio was around 11.8.
Just under three quarters had filled a prescription for a topical steroid or calcineurin inhibitor. Which means a quarter had not.
This is a condition that is treatable, that is monitorable, and that carries a consequence when it is neither.
Look, Once, In Good Light
Do this, and I am aware of what I am asking. Take a hand mirror, find decent light, and look properly.
Most women have never done this, which means they cannot describe a change, because they never knew the baseline.
You are looking for pale white patches, skin that looks shiny or thinner than the skin around it, loss of definition where the inner lips meet, and any split or fissure.
Book an appointment and use the words. I would like a vulval examination. Could this be lichen sclerosus.
Naming a condition changes the consultation. It moves you from a vague complaint to a differential diagnosis.
If there is any doubt, or any thickened, raised, ulcerated or non-healing area, ask about biopsy rather than waiting to see.
And if you are diagnosed, use the steroid exactly as prescribed, for as long as prescribed. Undertreating it out of caution about steroids is the most common failure, and it is the one that costs the most.
Wash with water or an emollient instead of soap. Keep your follow-up appointments, because the surveillance is part of the treatment.
It was never dryness. It was a skin condition with a name, waiting for somebody to say it out loud.
