Inside your vestibular system sit tiny crystals of calcium carbonate called otoconia. They rest on a membrane and their weight is how your brain knows which way is down.

The Crystals Are Made Of Calcium
Which is the part that should have been explained to every woman in her forties.
A systematic review in BMC Neurology in 2014 pooled seven studies covering 1,631 patients with this condition against 402 healthy controls.
Reduced bone mineral density was associated with both getting it and getting it again. Four of those studies looked exclusively at postmenopausal women. Osteoporotic patients recurred more and needed more repositioning attempts.
The authors were careful. They called the association possible and the evidence not strong.

Ask For The Test By Name
Do this. Say Dix-Hallpike. That is the positional test, and the horizontal canal version is the supine roll test.
It takes about a minute. You lie back with your head turned and the clinician watches your eyes. Either the spin reproduces with the characteristic eye movement or it does not.
You do not need a scan first. You need someone to move your head and look at your eyes.
Then Ask For The Manoeuvre
If the test is positive, the treatment is a repositioning manoeuvre, usually the Epley, performed in the room, immediately. It works by walking the crystals back out of the canal.
It frequently resolves in one or two attempts. Ask to be re-tested at the end of the appointment.
And refuse a vestibular sedative as the whole plan. It blunts the sensation and delays the thing that actually fixes it.
Vertigo lasting hours rather than seconds. New hearing loss or ringing in one ear. Double vision, a severe new headache, weakness or numbness, or unsteadiness that is not triggered by a change of position.
