Your Kidneys Run On A Clock

Antidiuretic hormone rises at night. Its job is to tell your kidneys to concentrate urine so you can go eight hours without needing to empty.

That is a circadian rhythm, and it is why you slept through the night for four decades without thinking about it.

Through the menopause transition that rhythm blunts. The night-time instruction to concentrate gets weaker.

At the same time the system that governs salt and water handling shifts its own timing.

So You Are Making More Urine At Night

Not sensing it sooner. Making more of it.

A 2024 review in the International Urogynecology Journal, led by Pauwaert and Everaert, sets this out. Estrogen deficiency drives abnormal salt and water diuresis at night through a blunted antidiuretic hormone rhythm and altered activation of the renin-angiotensin-aldosterone system.

Which means the problem is upstream of your bladder entirely.

Bladder capacity does fall as well, because that tissue is estrogen-dependent too. But if you are producing a daytime volume of urine overnight, no amount of bladder training will fix it.

And there is a loop hiding in this. The transition also dries your mouth at night. You drink to fix that. It comes back at four.

Measure It Before Anyone Guesses

Do this. Buy a cheap plastic measuring jug and keep a bladder diary for two full days, nights included.

Every time you go, write the time and the volume in millilitres. All of it, day and night.

Then add up your night volume and divide it by your total for the 24 hours.

If the night portion is more than about a third of your daily total, that is nocturnal polyuria. It is a different diagnosis from an overactive bladder and it has different treatment.

Say those two words at the appointment. Nocturnal polyuria, and here is my diary. That sentence changes what happens next.

Then Move The Fluid Earlier

Fluid pools in your legs during a day of sitting or standing. When you lie down it returns to circulation, gets filtered, and arrives as urine at two in the morning.

So empty the reservoir before bed rather than during it. Legs up above heart height for thirty minutes in the early evening. A walk after dinner. Compression socks on long desk days.

Reduce the evening salt, because salt is what obliges your kidneys to carry water out overnight.

And handle the dry mouth without volume. Swill and spit, suck something, humidify the room. Do not solve a dry mouth by drinking a glass of water you will meet again at four.

Your bladder was never the problem. Your kidneys lost their night shift, and nobody checked.
